<p>I did not explain myself very well. I meant I agree with your idea of  reducing the meds&#44; I think it`s a good idea. Halve them both&#44; and test extra  times to make sure you stay in the 5% club. I think you will stay in the  club&#44; because I also have faith in you.  Next A1c you`ll know whether to stay on the halved dose or drop the dose  completely. As you know&#44; exercise will help&#44; and so will dropping &nbsp;your  weight&#44; but at the moment I think it is likely that you are converting fat  cells into muscle cells&#44; so your weight looks the same. Is that right?  because you will know better than I can.  If I remember&#44; you are or were an body-builder? So you know more about how  to exercise&#44; but maybe I know more about how helpful with the insulin  resistance thingy the extra muscles can be. Then again&#44; maybe I don`t&#44;  because you sure are making good progress with this beast we got. <p>Great results&#44; well done! <img src='http://faqpcos.com/wp-includes/images/smilies/icon_smile.gif' alt=':-)' class='wp-smiley' />   Can&#8217;t comment on the drugs as I do not use those. I would give the diet side  some attention from what you say &#8211; probably feeling hungry and maybe not  enjoying the new foods as much as the old?  Makes it much harder to keep to the diet if you don&#8217;t enjoy what you eat. I  like some of the low calorie pasta&#8217;s they have got bulk&#44; are low calorie  (and carb.) with a low glycemic index.  &nbsp; Anyway&#44; good luck and keep up the good work.  &nbsp; Pete F </p>

<p>Yeah I know they&#8217;re not good numbers but I can&#8217;t get them down any further without the help of my specialist and I have another 3  weeks or so before I can see him.  &nbsp; I have a cr*p doctor who only gave me Gliclazide one a day &#8211; and he said it wasn&#8217;t enough&#44; but decided he wasn&#8217;t prepared to give  You see I also have PCOS (Poly Cystic Ovarian Syndrome) &#8211; makes it harder for me because I have hormones going all over the place&#44;  lots of pain and the insulin resistance is a bit worse than with diabetes alone. What is it you think the specialist will do to help you? &nbsp;i don&#8217;t even know what a specialist does.  and why does it take so long to see him? &nbsp;my old primary doctor was very old ide say close  to 80 even tho he looked in his 60&#8217;s. &nbsp;i seen some awards hanging on his wall  &nbsp;and from the dates on that he had to be either in his early 80&#8217;s or late 70&#8217;s.  he always had patients lined up but took a liking to my wife and I.  we lived a block away and soon as we would call he would say come on down.  i was very happy with him over the years &#8211; he seen me in my prime and my worst.  he was a little too relaxed and i noticed his nurse would stand over him with a watchful eye.  i guess &nbsp;he wasn&#8217;t what he used to be. &nbsp;i wish i could have seen him then and get another just  like him. &nbsp;i had a cyst removed from my back a few years ago. &nbsp;this thing grew in not out.  being i was even bigger at the time this thing was enormous. &nbsp;it was the size of big avocado  and was as ugly as this creature i seen in a movie years ago. &nbsp;i think the movie was  Big trouble in Tokyo or something. &nbsp;Goldie Horns boy friend was in it i forgot his name.  Kurt Russell.. yea that&#8217;s it.  it was a pretty good family type comedy.  well anyway it was that ugly. <p>Er Sushi&#44;  Metaphorically speaking you just caught me with my pants down. Insulin I  know&#44; sulphonylureas I know&#44; but very little about the other meds. The best  I can manage&#44; is  Metformin in about 17% of cases causes gastric upsets if started at a high  dose.  Metformin attacks insulin resistance&#44; so it is very unlikely to cause a  hypo.  Metformin helps to protect the heart&#44; and it might protect the arteries and  veins&#44; bit I don`t know for sure. <p>Metformin lowers blood glucose levels by decreasing excess production  of glucose by the liver and increasing the uptake of glucose by cells.  In the major UKPDS study&#44; it was the only drug that reduced  diabetes-related death rates&#44; heart attacks and strokes. &nbsp;Metformin  shuts off the liver&#8217;s over production of glucose. &nbsp;It reduces the  amount of insulin needed. &nbsp;I wouldn&#8217;t advise you to drop the Metformin  unless you were on a lower carb diet. &nbsp;I think you would be setting  yourself up for failure if you drop the Met while still having the  amount of carbs you do. &nbsp;But that is only *my* opinion. </p>

<p>Here&#8217;s a killer.  The medical profession doesn&#8217;t know either!  All that is known is its method of working&#44; not the exact mechanics. It  helps lower insulin resistance&#44; and supresses the liver-dump function.  Metformin doesn&#8217;t stimulate insulin production; it helps the body use  its insulin better. So it&#8217;s not a hypo-induing treatment.   and what someone has to do to be less dependent on it? </p>
<p>Lose weight&#44; exercise and avoid carbs in the first instance. But  remember you will need to monitor carefully&#44; and this condition can be  progressive. So coming off isn&#8217;t impossible by any means&#44; but don&#8217;t bet  on it being a forever thing.   you don&#8217;t have to get into the heavy tech stuff unless you want to.   i just want to understand it a little bit. </p>

<p>Why does my specialist take so long to see? &nbsp;He&#8217;s a specialist for Imperial College (one of the very best institutes in the world I believe) and you have to catch him when he&#8217;s in the country. &nbsp;I refuse to see someone which I believe is mediocre &#8211; I want to see the best&#44; I demand to see the best&#44; and I have private medical care so that enables me TO see the best. &nbsp;He&#8217;s not back in the country until August. &nbsp;So I have my first appointment with him then. They&#8217;re quite similar but we tend to get deposits   of insulin in the soft areas of our skin because there&#8217;s too much and it doesn&#8217;t move around &#8211; so giving me Gliclazide isn&#8217;t really   what I need &#8211; I need Avandia&#8230;&#8230;. but I&#8217;ll be discussing that with the specialist.   What is it you think the specialist will do to help you? &nbsp;i don&#8217;t even know what a specialist does.   and why does it take so long to see him? &nbsp;my old primary doctor was very old ide say close   to 80 even tho he looked in his 60&#8217;s. &nbsp;i seen some awards hanging on his wall   &nbsp;and from the dates on that he had to be either in his early 80&#8217;s or late 70&#8217;s.   he always had patients lined up but took a liking to my wife and I.   we lived a block away and soon as we would call he would say come on down.   i was very happy with him over the years &#8211; he seen me in my prime and my worst.   he was a little too relaxed and i noticed his nurse would stand over him with a watchful eye.   i guess &nbsp;he wasn&#8217;t what he used to be. &nbsp;i wish i could have seen him then and get another just   like him. &nbsp;i had a cyst removed from my back a few years ago. &nbsp;this thing grew in not out.   being i was even bigger at the time this thing was enormous. &nbsp;it was the size of big avocado   and was as ugly as this creature i seen in a movie years ago. &nbsp;i think the movie was   Big trouble in Tokyo or something. &nbsp;Goldie Horns boy friend was in it i forgot his name. <p>Can&#8217;t remember if Pete had moods on Metformin. Did you&#44; Pete? He could get quite  moody (unlike him) just prior to diagnosis. He did get stomach cramps on  Metformin &#8211; not squits&#44; but real&#44; awful stomach pains. He tried it for a month&#44;  then doc put him on Gliclazide. <p>My wife says I have quite rapid mood swings since diagnoses  I cant of course comment &#44; as being a man makes me right 150% of the time  <img src='http://faqpcos.com/wp-includes/images/smilies/icon_wink.gif' alt=';-)' class='wp-smiley' />   it wouldnt be fair on her <img src='http://faqpcos.com/wp-includes/images/smilies/icon_wink.gif' alt=';-)' class='wp-smiley' />   Okay I do recognise I have started losing my temper for what seems NO reason  at all&#44; since  I started on the metformin. I try to bite my tounge and count to 10&#44; which  normally helps. <p>Thanks guys&#44;  &nbsp; I knew that and I also told her that the Original name for PCOS is  Stein-Leventhal and that cysts on ovaries are just a symptom. I also told her that  I thought the doc was more curious about my uterus&#44; tubes and such for lack of  cycles. <p>I forgot to say that she was my old GP. My dad got a new job which meant  new insurence. The doctor I&#8217;m seeing now is really good&#44; and seems like  he&#8217;s open to anything. He sent me to a gyno for my PCOS but the  appointment was the day before Christmas Eve. and we all forgot. So we  plan to go to that doc and see what he says&#44; then end up having to go to  an endo anyway. It&#8217;s also suspected that I have CAH to complicate things  even more.  I&#8217;m always on the lookout for a new doctor though. In case the current one  falls through.  Thanks for the help. <p>Thank you for the advice&#44; really. I&#8217;ve already gone there. I went there  right after the Doc was on Dateline. The one I saw said I didn&#8217;t even have  PCOS and that I&#8217;m just fat and I needed to see a nutritionist. Even though  my free testosterone was elevated with the few tests he did run.  The people there treated me like crap basically. The doc was just looking  for people he could have in his study&#44; I wasn&#8217;t (I hope none of you take  offense to this.) hairy enough&#44; I didn&#8217;t have acne covering my face (I  actually have it on my rump which I think is actually worse because  sometimes I can&#8217;t even sit down.)&#44; and at the time I wasn&#8217;t fat enough.  Since then I&#8217;ve gained at least 50 pounds.  It took almost two months before I received my tests results and even that  took my mother calling threatening to call the medical director of the  hospital. After we did finally receive we had suspicions that he made some  of the tests results to appear normal when they actually weren&#8217;t. We took  them to my GP and she said to take them over again and see what the  results were this time. We did and they weren&#8217;t even close to being what  the doc at UIC wrote down. My GP said there was no possible way everything  could fluctuate as much as it did according to the results that doctor  gave.  Does anyone know of anyone else in the city of Chicago I could go see? I&#8217;m  beginning to run out of options rather quickly. Thanks. <p>It&#8217;s not always that easy to just go to doctor&#8217;s all the time. I have  another medical problem that doc&#8217;s have -no- clue what&#8217;s so ever about.  It&#8217;s what the astronauts get when they come back from space. NASA has no  clue about it. I&#8217;ve got double the annoying doctor&#8217;s&#44; un-needed tests and  stupidity! Plus&#44; the medicine that -can- possibly help the PCOS I can&#8217;t  take because it lower&#8217;s blood pressure which I can&#8217;t do because it&#8217;s  normally 40/60.  Also saying just go until you find a doctor that has a brain about this  isn&#8217;t easy because 1) people have jobs that they can&#8217;t take off work for.  2) don&#8217;t have the money to be spending on going to 5 different doctor&#8217;s in  hopes of finding one that might try and help. That&#8217;s what people have said  to me and my family about my other problem&#8217;s. There are doc&#8217;s in other  states that have better understanding of it&#44; but my family doesn&#8217;t have  the money to be spending on train fare (my brother and I can&#8217;t fly.)  hotels plus the doctor visit which I&#8217;m sure the insurance wouldn&#8217;t pay  for. So were stuck in Chicago&#44; where no one has a clue.  So saying just keep going to doctor&#8217;s isn&#8217;t as easy as you make it sound.  Spending $4000 isn&#8217;t something everyone can do&#44; PCOS doesn&#8217;t effect just  the rich. (yes to be able to spend $4000 on going to doctor&#8217;s does seem  rich to me.)  Grace </p>

<p>I was told that in my case it was the IC/PCO causing the weight gain.  i was a small to normal child before i hit early puberty and began to gain  weight uncontrollably. &nbsp;At the same time&#44; I developed acne&#44; oily skin and hair&#44;  mood swings&#44; had menstrual difficulties from the get-go. &nbsp;  After developing these symptoms&#44; I gained weight continuously for over 20  years&#44; even on weight loss diets. &nbsp;The only time I did not gain weight was on a  500 calorie a day liquid fast program.  When I look at pictures from childhood&#44; at age 6 I was a normal&#44; smallish  child&#44; and at age 8&#44; I was fat&#44; had breasts&#44; acne ridden&#44; and horrendously  greasy hair. &nbsp;The change was so dramatic and so drastic. &nbsp;  Tracy </p>

<p>Okay &#8212; I just got back from &nbsp;visiting with the gyn nurse and asked if my  endocrine dr. had sent the results of my lab (he hadn&#8217;t) so I had to explain  that I wanted to start clomid today (day 5) and that I am on Glucaphage (which  is new since I&#8217;ve seen gyn. last). &nbsp;The nurse ran in and asked gyn whether I  should start the clomid today and gyn told her &quot;NO!&quot; and since this PCOS  diagnosis and Glucaphage that I will need to get my sugars under control and be  on insulin injections before and during pregnancy!!!!! &nbsp;WHAT?!?! &nbsp;My glucose is  91 with reference range 60-114. &nbsp;There was no insulin test done so I don&#8217;t know  about that part. &nbsp;I had gestational diabetes with my first and only pregnancy  with a 9lb. 14oz. son delivered my c-section. &nbsp;She know all of that but I am  really doubting that she has a clue about the PCOS stuff. &nbsp;She was calling it  Syndrome X for awhile until I said&#44; &quot;I&#8217;m pretty sure that&#8217;s related but  different from PCO&quot;. &nbsp;She agreed later!! &nbsp;Scary! &nbsp;Anyway&#8230; any similar  experiences? &nbsp;Am I really going to have to take insulin injections? &nbsp;I thought  once you started that stuff you were on it for good? &nbsp;The more I talk about it  the more scared I get. &nbsp;Thanks everyone. <p>Hi Debbie&#44;  No wonder you were scared! &nbsp;Why do they want to start with something as  drastic as shots already? &nbsp;I have heard that lots of people have tried low  carb diets and it has brought their insulin under control. &nbsp;I think I would  try this before something drastic like shots. &nbsp;If you really are  uncomfortable with your Dr&#44; I would strongly suggest you switch to someone  you will be more comfortable with. The use of insulin lowering meds to control/alleviate PCOS is rather new (2  years)&#44; and is still unapproved by the FDA. &nbsp;Is one of those &quot;off label&quot; uses&#44;  for now.  &lt;&lt; If I had been taking that drug&#44; is there any chance that I might have become  pregnant?  Is possible&#44; but no guarantees. &nbsp;Depends on if you have other factors&#44; how you  respond to the meds&#44; etc. </p>

<p>Hi&#44; I have pcos too and am currently on metformin.  There is a chance that you could have conceived while on the met. due to the  fact that a large majority of women with insulin resistance and PCO resume  normal ovulatory cycles&#44; such as in my case.  I have been on it three months and have ovulated each month&#44; although I am not  pg yet.  There also could be other factors involved with you conceiving so even if the  met did work for you it doesnt necc.mean you would get pg.  The reason why met was never mentioned to you awhile ago&#44; was because it is  very new and experimental. Most dr&#8217;s don&#8217;t really know much about using insulin  lowering drugs to treat pco&#44; the good news is that some do.  I was blessed to have a RE who knew about it and put me on it. Other dr&#8217;s you  have to hound&#44; show studies to and educate them about what you know to get on  it.  It is not too late for you to see your dr. about the met. Also&#44; some people (like with PCOS) need their LH to go up much higher than  normal&#44; for it to be a &quot;true surge&quot; for them. &nbsp;For a normal person&#44; an LH level  of 15 can be a surge&#8230;&#8230;for someone with PCOS&#44; LH of 15 can be their  baseline&#44; and a &quot;surge&quot; for them could need to be a lot higher&#8230;..like 30+.  OPK&#8217;s test for LH = 20.) </p>
