Gestational Diabetes – Twin Pregnancy – HELP!!

Question:

I went into the hospital at 24 1/2 weeks pregnant with twins with pre- term labor.  At 28 weeks, I was given the 3 hour glucose tolerance test.  The results were fasting glucose of 97, 1 hour of 172, 2 hour of 134, and 3 hour of 147.  Urinalysis was negative.  No treatment (insulin, diet, etc.) was started.  I was, however, told that a follow- up test would be completed at 32 weeks. The follow-up test was not completed.  My babies were born at 33 2/7 weeks weighing 6 pounds 1 ounce and 5 pounds 4 ounces.  The were very large for being so premature.  They were also hypoglycemic, spending 2 1/2 and 5 weeks in the hospital after birth. My question is, what is the maximum glucose levels for the 3 hour glucose tolerance test?  My preliminary research suggests that treatment should have started at 28 weeks.  However, I don’t know if the twin pregnancy would change the levels that warrant treatment. My pregnancy book says: Fasting: up to 105mg/dl 1 hour: up to 190mg/dl 2 hour: up to 165mg/dl 3 hour: up to 145mg/dl It doesn’t say anything about multiple pregnancy. All your numbers, except the 3-hour one, were fine.  And the 3-hour was just a hair above the line (147 as opposed to 145).  (Though it is interesting that it was higher than the 2-hour…)  So it’s not all that surprising that nothing was done at that point. As a fellow twin mom, I would say to just be thankful that your little bundles are here.  Mine are 5 now, and I am still questioning what I could have done differently, what the Dr could have done differently (there were some major things!), etc. in order to have avoided the life and death struggle we went through (not related to blood sugar or PCOS).  They are fine now, no permanent damage.  I am still struggling to accept it all.  But with time I can see that things worked out the way they were supposed to, and there is nothing like a baby or two in NICU to make you appreciate the value of life.  Enjoy them!  I know this doesn’t answer your question, but I hope it helps.
