Lupron Flare

Question:

Could someone please explain what the Lupron Flare protocol is and why it is supposed to improve egg quality?  I make enough eggs, just poor quality embryos. Thanks Tricia IVF/ICSI #4 in November

First, I strongly suggest you do a websearch (try www.asrm.org) to get some *real* info. Briefly, it is my understanding that Lupron Flare consists of a single injection of Lupron, which causes a flare/surge/sharp rise in LH and FSH.  This gives your system a little boost.  The Flare dose is given the day before starting gonadotropins. Flare may be chosen over Downregulation, to avoid oversuppression, which can happen with Lupron.   I don’t know why it would improve egg quality.  (I can see how it would help with quantity, tho).  For the most part, egg quality is inherent..your eggs are your eggs….can’t change ‘em.  HOWEVER, different protocols can be more or less detrimental to your eggs, depending.  Been there. I assume you’re doing IVF.  If so, perhaps your RE feels that a downregulation protocol would be detrimental to your eggs somehow, and therefore wants to try something different.  How, I really don’t know.  A lot of it is trial and error.  Best of luck, -Kay PCOS, primary, trying 15 yrs

My understanding of the Lupron flare protocol is when you stop Lupron altogether  when you start the stimulation meds.  It’s also called "Stop Lupron Protocol." Mary Lind

 << It’s also called "Stop Lupron Protocol." I believe these are two different things.  With Stop Lupron, you take the Lupron daily, for a week or so, and stop prior to stim meds. With Lupron Flare, you take just a single shot of lupron, prior to stims. I suppose Flare could be considered a type of "stop"…you *do* stop the Lupron before taking stims.  However, the term "Flare" refers to the *flare*  (or surge) of LH and FSH that the single Lupron bolus provides.  That is the whole purpose of using a Flare….to give an LH/FSH surge. The purpose of Stop, I believe (hopefully correctly), is not for the "flare effect" but is to minimize whatever downregulation might occur. That said, in infertility, sometimes names are ambiguous, so maybe I"m wrong. You say po-TAH-toe, I say po-TAY-toe…?
